________ Birth Certificate of the minor/child (from NSO)
________ Marriage Certificate of Parents (from NSO)
________ Notarized affidavit of Consent from Parents authorizing a particular person to accompany the minor for his / her travel.
________ Notarized affidavit of Support of Sponsoring person indicating Employment/salary. Other proofs of income, Certificate of Employment, business permit, etc.
________ 4pcs. 2x2 or passport size picture of the minor
________ Photo Copy of Passport or visa of travelling companion
For minor who is travelling alone 13 years old and above
________ Notarized Affidavit of Consent authorizing the minor to
travel alone, likewise stating that the minor shall be fetched at the airport by parents/sponsor
NOTE: Minors below 13 years old are not allowed to alone/unaccompanied

- BRGY.CERTIFICATION pagpapatunay na kayo ay residente ng barangay na nabibilang sa mahirap na pamilya.
- MEDICAL CERTIFICATE/ABSTRACT nakalagay ang pangalan ng pasyente,may petsa,nakapirma ang doktor sa ibabaw ng nalimbag niyang pangalan at nakalagay ang license number.
- RESETA NG GAMOT (depende sa paggagamitan) kagaya ng nabanggit sa no.2, nakalagay din ang ganoong impormasyon.
- DEATH CERTIFICATE AT FUNERAL CONTRACT (depende sa paggagamitan).
- PERSONAL LETTER kung sa PCSO hihingi ng tulong.
